Mental Health Services in BC: Finding a Counselor or Therapist

A guide to accessing mental health services in British Columbia — counseling, therapy, psychiatry, MSP coverage, free resources, and how to find mental health support on mydoctorbc.ca.

Mental health is health. Yet for many British Columbians, finding the right mental health support — a counselor, therapist, psychologist, or psychiatrist — remains a confusing and frustrating process. Who provides what? What does MSP cover? Where do you start if you do not have a family doctor? This guide cuts through the complexity and explains how to access mental health services in BC in 2026. Understanding Mental Health Providers in BC One of the biggest sources of confusion about mental health services in BC is the different types of providers. Each has different training, different services, and different coverage under MSP. Psychiatrists are medical doctors (MDs) who specialize in mental health. They can diagnose mental health conditions, prescribe medications, and provide therapy. Psychiatrist visits are fully covered by MSP. However, you generally need a referral from a family doctor, nurse practitioner, or other physician to see a psychiatrist. Wait times for psychiatry can be long — months in many cases. Psychologists hold doctoral degrees (PhD or PsyD) in psychology. They provide assessment, diagnosis, and therapy (including cognitive behavioural therapy, exposure therapy, and other evidence-based treatments). Psychologist visits are not covered by MSP. Sessions typically cost between $180 and $250 per hour. However, many extended health insurance plans through employers cover some psychologist visits. Registered Clinical Counsellors (RCCs) are trained therapists who provide counseling and psychotherapy. RCC sessions are not covered by MSP but are often covered by extended health insurance. Fees typically range from $120 to $180 per session. Registered Social Workers (RSWs) who provide clinical counseling offer similar services at comparable rates. Family doctors and nurse practitioners can diagnose mental health conditions, prescribe psychiatric medications (antidepressants, anti-anxiety medications, sleep aids), provide brief counseling, and refer you to psychiatrists and other mental health specialists. Visits to family doctors and NPs are fully covered by MSP. What MSP Covers MSP covers visits to family doctors and nurse practitioners for mental health concerns. This includes assessment, diagnosis, medication prescribing, and brief counseling. MSP covers psychiatrist visits when referred by a physician or NP. MSP does not cover psychologists, counselors, or therapists in private practice. There are exceptions and supplementary programs. BC has invested in free counseling programs and community mental health services (described below) that provide no-cost therapy regardless of MSP coverage. If you have extended health benefits through your employer, check your plan — many cover a number of psychologist and counselor sessions per year. Free and Low-Cost Mental Health Resources in BC BC has several free mental health resources available to all residents. 8-1-1 Mental Health Support: Call 8-1-1 (HealthLink BC) and ask to speak with a mental health professional. Available 24 hours a day at no cost. Crisis Line: If you are in crisis, call or text 9-8-8 (the national suicide crisis helpline) for immediate 24/7 support. Foundry BC: For youth and young adults aged 12 to 24, Foundry centres across BC provide free, walk-in mental health services, substance use support, and primary care. BounceBack: This is a free, evidence-based CBT (cognitive behavioural therapy) program available to BC residents through their family doctor or NP. BounceBack provides guided self-help workbooks and phone coaching for people dealing with depression, anxiety, stress, and worry. Ask your doctor for a referral. Access to Free Counseling Through Primary Care Networks: As part of BC's primary care reform, many primary care networks now include counselors and social workers on staff. If you are attached to a family doctor or NP within a primary care network, you may be able to access free counseling sessions through that network. Ask your provider about availability. Community Mental Health Teams: Each health authority in BC operates community mental health teams that provide free services for people with moderate to severe mental illness. These teams include psychiatrists, nurses, social workers, and occupational therapists. Referrals typically come through your doctor or NP, or through a hospital emergency department. How to Find Mental Health Support If you have a family doctor or nurse practitioner, start by talking to them. They can assess your needs, prescribe medication if appropriate, refer you to a psychiatrist, connect you with BounceBack or other free programs, and refer you to a counselor within their primary care network. If you do not have a family doctor, start by searching Find a Doctor BC at mydoctorbc.ca for clinics that list mental health services. Many family practices and community health centres include mental health support as part of their care. Walk-in clinics and UPCCs can also provide initial mental health assessment and prescriptions. For private therapy, directories like the BC Association of Clinical Counsellors (bcacc.ca) can help you find registered providers. When choosing a private therapist, ask about specialization, fees, and whether they offer sliding-scale pricing. Mental Health and the Doctor Shortage The family doctor shortage in BC has a direct impact on mental health access. Without a regular doctor, it is harder to get referrals to psychiatrists, access to free programs like BounceBack, and consistent medication management. This makes finding a primary care provider even more important if you have ongoing mental health needs. Register with the Health Connect Registry by calling 8-1-1. When you register, you can indicate that you have mental health concerns — this information helps the registry match you with an appropriate provider. Also search mydoctorbc.ca regularly for clinics accepting patients in your area. Tips for Patients You do not need to be in crisis to seek help. If you are feeling persistently anxious, sad, overwhelmed, or are having trouble with sleep, concentration, or daily functioning, reaching out is the right step. Be honest with your provider about your symptoms. Mental health assessment relies heavily on what you share. If cost is a barrier, ask about free programs first. 8-1-1, BounceBack, Foundry (for youth), and community mental health teams are all free. If you are prescribed medication, follow up regularly with your prescribing doctor or NP. Mental health medications often need adjustment over the first few weeks. Therapy takes time.
